This is a launch-ready operational template, not a representation that an attorney has reviewed it.
Permitted use
Use NPI Roster to work with provider-roster and other lawful public business information. Do not upload PHI, patient records, executable content, or data you lack authority to process.
Important limitations
NPI Roster summarizes public CMS/NPPES information. It is not credentialing, licensure verification, exclusion or sanction screening, medical advice, legal advice, or a determination of provider eligibility. You remain responsible for decisions and required verification.
Accounts and billing
Paid subscriptions renew monthly until cancelled. Plan limits apply by billing period. Downgrades preserve account access but can pause monitoring and prevent new saved rosters above the lower limit; data is not silently deleted.
Availability
Public upstream services can be incomplete, unavailable, or delayed. Partial results and timestamps are provided so you can assess a validation. We may change reasonable rate and file limits to protect the service.
Termination
You may stop using the service and delete your data. We may suspend abusive, unlawful, or security-threatening use. Warranty, liability, governing-law, and dispute language should be completed with counsel for the launch jurisdiction.
